Patient Information

Patient Information

Patient care is of the utmost importance here at L. Ehrlich & Associates Medical Clinic. For your convenience, we have created an online system so that you are able to access your patient information from the comfort of your own home.

This system includes our patient portal for scheduling and billing, the ability to submit forms, and having access to relevant information pertaining to our practice. Here, you can learn helpful patient information related to our policies and medical records in order to make your healthcare experience more efficient.

Our Policies for Patients

There are a few policies that patients need to follow at L. Ehrlich & Associates Medical Clinic in order to help build an efficient and safe system. For instance, if you are looking to have your prescription refilled, you need to contact your pharmacy. From there, they will contact our office to confirm the refill, and they will then inform you when your medication is ready for pick up. If you are out of refills on a prescription, you will have to schedule an appointment with your physician to renew the script. Some insurance companies require that you have a referral request sent to them. We are happy to do this for you, but you will have to provide us with the right information. Contact your insurance provider and have them send you the proper paperwork. Typically, these requests are a simple sheet or form that you will have to partially fill out before handing it over to us. From there, we will handle the rest.

If you have had lab test run and are awaiting the results, you have to contact our office directly. We can send you a hard copy of the results, if that is what you prefer. However, if you need to speak to your doctor concerning the results, you will have to schedule an appointment. For further questions about our practice’s policies regarding patient information, please contact our office via phone or email.

Medical Records in Houston

There are several reasons why someone would need a copy of their medical records. However, because we use an electronic medical records (EMR) system, you have to request for a copy of your medical records through our office. This procedure ensures the safety of all of our patients’ and practice’s private information. If you need a copy of your medical records, please contact our office for more information.
